Vacancy for young person’s social prescribing link worker

If you are enthusiastic and self-motivated individual, whose passion is to inspire and empower young people, we have just the position for you.  We would love you to join our Thr1ve Social Prescribing Service, supporting young people 1:1 to make positive changes to their lives.

This role will require you to adopt a person-centred approach to tackling the issues Young People are facing. Link workers often work one-to-one with Young People out in the community, helping to assess needs, and set goals, and identify existing groups, services, and organisations in their area.

Supporting the Young People by removing barriers to participation, and helping build their confidence and resilience, getting them to a place where they feel confident enough to thrive on their own.

As a result of receiving additional funding, we are now looking to expand our team of link workers to provide even more community-based support for 14–18-year-olds (up to 25 with additional needs).

Employed by Chesterfield Football Club Community Trust, Thr1ve Social Prescribing link workers cover the following areas: Chesterfield, North-East Derbyshire, Bolsover, Amber Valley, North Derbyshire Dales and Glossop.

Each link worker has a designated area but may be expected to cover other areas on occasion.

Ideally you will have worked in a supportive role with young people before and will have a great understanding of the many and complex challenges young people are currently experiencing.Positions Available:

1.0 FTE –Social Prescribing Link Worker for Young People £25,500

1.0 FTE – Senior Social Prescribing Link Worker for Young People £27,500
