How to Refer

Who can be referred?

We have tried to remove as many barriers to accessing our service as possible. However, there is some criteria for being eligible for our service.

Any young person aged 11-18 can be referred or up to aged 25 with an EHCP.

As our service is mainly limited to Chesterfield, Bolsover and North Hardwick, they must also:

  • Have a home address in this area
  • Be on role at a school in this area
  • Be a looked after child or care leaver living in this area
  • Not be in Employment, Education or Training (NEET) and living in this area;
  • Be a patient within North Hardwick & Bolsover PCN
  • Attend Chesterfield FC Community Trust

Who can make a referral?

Anybody can make a referral to us. You do not need to be a health care or social care professional. We are happy to receive self-referrals as well as referrals from parents and guardians, school nurses and GPs, or any other professional working with the young person. We do request, however, that the young person be made aware of their referral and the reason why.
