Vacancies for children and young people’s social prescribing link worker

If you are an enthusiastic, self-motivated support worker, whose passion is to inspire and empower children and young people, we have just the challenge for you.  

Derbyshire Voluntary Action and Chesterfield Football Club Community Trust have been working in partnership since April 2021 to develop social prescribing for young people in Chesterfield.  

Thanks to new support from Derbyshire Community Healthcare Services and North Hardwick and Bolsover Primary Care Network, the partners are now looking to extend the initial pilot project through to March 2023 and expand their team of link workers to provide even more community-based support for 12-18-year-olds.

Three posts are available:

1 x 1.0 FTE Link Worker – Bolsover and Hardwick PCN

1 x 1.0 FTE Link Worker – Bolsover and Chesterfield

1 x 0.6 FTE Link Worker  Bolsover and Chesterfield

Employed by Chesterfield Football Club Community Trust and based at the Technique Stadium’s Community Hub, you’ll adopt an holistic, person-centred approach to your caseload, co-designing action plans to improve wellbeing and health, based on ‘what matters’ to young people and their individual strengths and needs – and supporting them to achieve their goals.  

Ideally you will have worked in a supportive role with young people before and will have a great understanding of the many and complex challenges young people are currently experiencing.

This is a 12-month pilot project to test the success of a social prescribing approach with 12-18-year-olds in Bolsover and Chesterfield. The post is subject to an enhanced level DBS check.
